INSURANCE WE ACCEPT:
We accept most major medical insurances and EyeMed vision plan. Please be aware that within these insurance plans there are some policies that we do not accept. Questions about coverage will be discussed when you call to make your appointment.  
 
VISION PLANS VS. MEDICAL PLANS
About Your Insurance:
There are two types of health insurances that will help pay for your eye care service and products. You many have both and our practice accepts both.
They are:
Vision care plans (such as EyeMed, Anthem Blue Vision)
Medical insurance (such as Anthem, Medicare, Aetna)


Vision Care Plans
Vision care plans (such as EyeMed) only cover routine vision exams along with eyeglasses and contact lenses. These plans only cover a basic screening for eye disease. Vision plans do not cover diagnosis, management or treatment of eye diseases (examples: diabetes, glaucoma, dry eye disease, cataracts).

Medical Insurance
Medical insurance must be used if you have any health problem that has ocular implications. The doctor will determine if these conditions apply to you. Some conditions are determined by your case history.

If you have both types of insurance plans, it may be necessary for us to bill some services to one plan and other services to the other.

We will bill your insurance plans(s) for services if we are a participating provider for that plan. If some fees are not paid by your plan, you will be responsible for any unpaid deductibles, co-pays or non-covered services as allowed by the insurance company. 

CONTACT LENSES
Contact lenses are considered a medical device and require an annual contact lens prescription. This is not the same as a glasses prescription. Every contact lens patient will be charged an evaluation fee annually. This includes re-examining your prescription and ensuring your contacts are still fitting properly. It includes any trials you may need for fitting purposes, but does not cover your annual supply of contact lenses. Contact lens fittings are not covered by medical insurance plans. Sometimes vision plans have contact lens fitting benefits, but the benefits vary with different vision plans.
