NEW POLICY!! Effective September 1, 2024 any new or established patients who fails to show up for an appointment or cancel/reschedule without providing at least 24 hours notice will be charged a $40 no show fee. This fee will be billed directly to the patient, not to the insurance company.
Accepted Insurances:
- Most major medical insurances and EyeMed vision plan are accepted.
Not Accepted Insurances:
- Ameritas, Cigna Vision (VSP), Davis Vision, FED VIP, Guardian Vision & UHC Vision (Spectera)
Vision Plans vs. Medical Plans:
- There are two types of insurances accepted: vision care plans and medical insurance.
- Vision care plans (e.g., EyeMed, Anthem Blue Vision) cover routine vision exams, eyeglasses, and contact lenses. They do not cover diagnosis or treatment of eye diseases.
- Medical insurance (e.g., Anthem, Medicare, Aetna) must be used for eye problems with medical implications.
- If you have both types of insurance, some services may be billed to one plan and others to the other.
Insurance Billing & Responsibility:
- We will bill your insurance plan(s) if we are a participating provider.
- You are responsible for any unpaid deductibles, co-pays, or non-covered services as allowed by the insurance company.
Contact Lenses:
- Contact lenses are considered a medical device and require an annual prescription separate from glasses.
- An annual evaluation fee is charged for contact lens patients, including prescription re-examination and fitting purposes.
- Contact lens fittings are not covered by medical insurance plans, but some vision plans may offer benefits (such as Eyemed).
